I am very pleased that you have chosen to join our firm and I know that this is the beginning of a mutually beneficial association. Download the Substitute Employee Application Packet that contains the documents that are required for your employment with Temporary School Staff. Completion of these forms will ensure you are paid accurately and on a timely basis. Once completed please mail or fax these documents to the address or fax number indicated below. You must submit a time sheet to us at the end of the work period. Download the Time Sheet for hourly employees to record the time and location worked during the period. Fax your time sheet to 248-360-9662. Time sheets must also be signed by the appropriate school district administrator. You should also complete and submit the following EZ LABOR ONLINE TIME CARD MANAGER 2009 TSS PAYROLL DATES 2008-09 Employment Eligibility Certificate Federal tax form, Michigan tax form Direct Deposit Form For your deduction calculations Circular-E tax guide (See pages 40-59) in the link below http://www.irs.gov/pub/irs-pdf/p15.pdf (if you want your pay deposited directly into your bank account.) Submit your time sheet either bi-monthly or monthly by the 15^th and/or 30^th of each month. Pay dates are the 15^th and 30^th of the month. Here is how it works: Ø Time submitted by the 15^th is paid on the 30^th Ø Time submitted by the 30^th is paid on the 15^th If the 15^th or 30^th falls on a Saturday you will be paid on Friday the 14^thor 29^th. If the 15^th or 30^th falls on Sunday, you will be paid on Monday the 16^th or 31^st (or 1st in those months containing 30 days). In February, the pay date for the 30^th is on or about March 1^st.
